Vitality Program

How does the Vitality Program work?

Our staff completes a vitality assessment, a wellness assessment that looks at your health, physical and cognitive ability, quality of life and more. From there, you’ll get a personalized Vitality Plan with a variety of recommendations, from activities to classes, that optimize your health and independence.

What other types of health care services are included?

Residents have access to many services, including assistance with health care coordination, daily check-ins, 24-hour on-call emergency support, access to our on-site health clinic for nursing support and assistance with transportation to medical appointments.

Are residents required to participate?

Participation is entirely voluntary, but more than 95 percent of our residents choose to participate and recognize the extraordinary value in the services this program offers.

Assisted Living

What type of care does assisted living provide?

Assisted living care means we empower our residents to maintain as much independence as possible by providing just the right amount of support. If you need a helping hand with daily tasks, such as dressing, bathing, taking medication or just getting around, we can provide as much or as little assistance as you need.

What are some examples of the community amenities?

Assisted living residents have access to all community amenities at The Village, as well as a full monthly calendar of social and life enrichment designed just for them. Select apartments offer kitchenettes and all assisted living communities feature recreational space and dining venues.

Are transportation services available?

We provide scheduled transportation for residents; our associates organize numerous outings every month for shopping, special events, social outings and doctor appointments.

How large are the assisted living residences at The Village?

Assisted living residences are available as studio and one-bedroom private apartments in a range of sizes from 355 to 529 sq. feet. Each residence features its own private bath and all utilities and basic cable services are provided, except local telephone. To ensure immediate staff response, residents wear pendants that alert staff that help is needed at the push of a button.

Memory Support

How does memory support differ from other levels of care?

Our memory support communities have been thoughtfully and carefully designed to ensure that residents feel safe and secure. Assistance is provided under the direction of a licensed nurse along with a specially trained staff dedicated to the well-being and comfort of residents. Ivy Lane’s unique neighborhood design affords individualized attention and social interaction in a homelike setting to minimize agitation that often accompanies those affected by memory impairment. Azalea Trace’s enclosed garden offers a safe space for outdoor activities and family visits along with a walking path with points of interest along the way. Each encourages the highest level of independence while focusing the individual strengths and preferences of our residents.

What kind of special training do associates have?

Our associates are trained in one of the country’s most respected dementia care programs – the Positive Approach™ to Care, an approach developed by Teepa Snow, an Occupational Therapist and one of American’s leading experts on dementia. This program focuses on the abilities that remain versus what has been lost. Memory support at The Village places people before tasks and nurture relationships that recognize the importance of privacy, choice, dignity and purpose.

Are transportation services available?

We provide scheduled transportation for residents; our associates organize numerous outings every month for shopping, special events, social outings and doctor appointments.

What are the residences like?

Ivy Lane offers studio and one-bedroom apartments with a private bathroom; Azalea Trace features two neighborhoods of private rooms, each with a half-bath. All utilities except telephone are provided.

What kinds of amenities are included?

Residents enjoy three chef-prepared meals every day, served family-style. We also provide housekeeping, daily bed-making and personal laundry services. Residents have access to activity common spaces designed to promote social interaction. Additionally, we provide interactive and socially stimulating programming specifically designed for those with memory loss.
